  Submit the questionnaire by:
  1. Take a photo of each page and send to our office at (252) 940-8296
  
  2. OR save & email the completed PDF to pamlico409@aol.com)
  
  Once submitted, please proceed to step 2! If you have any questions about Step 1, please call our office.

Step 2:

• Use your smartphone to take a photo of the following related tax forms and indentification:
  
  • W2, 1099 Form, or Unemployment Form
  • Copy of Drivers License
  • Copy of Social Security Card(s)
  
  **Please be sure that your information is clearly visible in the photograph.**
  

Step 3:

• Use your smartphone to take a selfie of you, along with the following forms of identification: Text your selfie(s) with your identification (from Step 3) to the Pamlico Tax Service office at:
  
  (252) 940-8296
  
  **We will confirm receipt of your information.**

Step 4:

• Collect your money!
  
  Once your documents have been submitted and received, we will process your tax return Once your return is completed, your money will either be deposited directly into your bank account, or we will provide you with a check.
